Comprehensive wound evaluationDetailed wound assessment, measurement, documentation, risk-factor review, and individualized care planning.
Standard wound careCleansing, dressings, moisture balance, compression/offloading coordination, patient and caregiver education, and ongoing monitoring.
DebridementRemoval of non-viable tissue when clinically appropriate to support the wound-healing process.
Negative pressure wound therapy (wound VAC)Evaluation, management, troubleshooting, and coordination of NPWT when appropriate.
Minor bedside proceduresCertain wound-related procedures may be performed in the home or facility setting when appropriate, safe, and clinically indicated.
Advanced wound care optionsAdvanced therapies, including cellular/tissue-based products or skin substitutes, may be considered when medically necessary, when the wound meets clinical criteria, and when standard wound care has not achieved expected progress.
Infection monitoring and escalationMonitoring for signs of infection or deterioration, with referral or escalation when a higher level of care is needed.
Surgical escalationWhen a wound requires a higher level of intervention, PWS can coordinate with Prestige Foot & Ankle Center and hospital-based resources for surgical evaluation or treatment.
Care coordinationCommunication with referring providers, home health, facilities, caregivers, hospitals, and community resources.

Advanced options are a tool, not the headline.

Advanced wound techniques, such as skin grafts, may be appropriate for certain chronic or complex wounds that do not respond as expected to standard wound care. These options are not one-size-fits-all. They are considered only after clinical evaluation, documentation of wound history, assessment of underlying causes, and review of medical necessity and payer requirements.

Escalation Pathway

When a wound needs more than wound care.

Not every wound can be managed conservatively. When surgical evaluation, vascular intervention, or hospital-level care is needed, our team coordinates directly with Prestige Foot & Ankle Center and area hospitals to keep the transition smooth and the referring provider informed.
